Kratom: The Controversy Surrounding a Plant-Based Supplement

The discourse around kratom has emerged as a focal point in various state legislatures, sparking debates over whether to regulate or entirely ban its use.

My journey in health and wellness began in the mid-90s when my parents recognized the importance of fitness and enrolled us in a local gym. I started exercising regularly and delved into various workouts and natural supplements.

Among the frequent visitors at the gym was an individual who was keen on sharing his knowledge. He offered insights into proper lifting techniques, optimal repetitions for achieving desired results, and occasionally discussed the supplements he was using. One day, he introduced me to a substance called ephedrine.

Curious, I asked him about its purpose. While I can't recall his exact words, he described it as a stimulant that people often used for enhancing energy levels or weight loss. What stood out vividly in my memory was his cautionary note: “You need to be careful; it can disrupt your heart rhythm.”

For those unfamiliar, ephedrine is closely related to pseudoephedrine and phenylephrine, both of which are bronchodilators found in decongestants like Sudafed. As stimulants, they can indeed have adverse effects on the heart, including increased blood pressure, irregular heartbeats, and even heart attacks. These dangers led the FDA to prohibit ephedrine in supplements by 2004.

The challenge with products labeled as ‘natural’—like ephedra, an evergreen shrub—is that they often lack consistent quality control. The variability in active ingredient concentrations can increase health risks for consumers.

This brings us to kratom, another ‘natural’ supplement that has been under scrutiny by legislative bodies. But what exactly is kratom, and why is it facing such intense examination?

Kratom, derived from the leaves of the Mitragyna speciosa tree found in Southeast Asia, can be consumed in multiple forms. Fresh leaves can be chewed, dried leaves can be brewed into tea, or it can be manufactured into pills or capsules. Each method of consumption can significantly affect how the body processes this substance.

Understanding this is essential, as kratom's effects are heavily dose-dependent and believed to interact with opioid receptors. At lower doses, it serves as a stimulant, offering an energy boost; however, at higher doses, it can act as a pain reliever or sedative. These dual characteristics have drawn interest from both consumers and lawmakers. While some question the medicinal benefits of kratom, others express concerns over health risks, quality assurance, and the potential for misuse.

Currently, kratom is banned in several states, including Alabama, Illinois, Wisconsin, Tennessee, and Indiana. Other states are contemplating regulatory measures. For instance, in Florida, Senate Bill 1076 aims to prohibit the sale of kratom to individuals under 21 and to implement quality-control regulations for kratom products. Meanwhile, Ohio is exploring regulations for kratom, which is often used for pain relief and opioid dependency, although some experts deem it hazardous.

Implementing regulation poses significant challenges. The nutritional supplement industry is diverse, with substantial discrepancies in quality control, ingredient sourcing, and labeling practices, placing the onus on consumers to be vigilant.

Proceed with Caution

As the world becomes increasingly interconnected, knowledge about medicinal products and herbal remedies, once limited to specific locales, is now widely accessible. Consumers seeking alternatives to conventional pharmaceuticals are turning to herbal supplements and natural remedies.

However, it is critical not to equate ‘natural’ with ‘safe’. Numerous plants and trees contain potent compounds that can significantly influence bodily functions and homeostasis.

Kratom is no exception.

Medical authorities, including the Mayo Clinic and the National Institute on Drug Abuse, remain skeptical of kratom's safety. The Mayo Clinic has labeled kratom as unsafe and ineffective, while the National Institute on Drug Abuse states, “There is no scientific evidence supporting kratom's efficacy or safety for treating opioid addiction or pain, necessitating further research.”

I anticipate that clinical studies will be the next logical step. Expanding options for pain management could be beneficial, provided these alternatives prove effective and less addictive than opioids.
