In August, a groundbreaking randomized clinical trial demonstrated that administering oral calcifediol (commonly known as vitamin D3) significantly lowered the risk of needing intensive care from 50% (13 out of 26 patients) to just 2% (one out of 50) among those infected with COVID-19. “While the trial was small, the results regarding ICU admissions were so striking that they hold significant statistical weight,” highlighted a letter published in The BMJ.

This month, another clinical trial conducted in India focused on vitamin D3's effects in COVID-19 patients and was released in the Postgraduate Medical Journal of the British Medical Journal. Titled “Short term, high-dose vitamin D supplementation for COVID-19 disease: a randomised, placebo-controlled, study (SHADE study),” it reinforces the necessity of healthy vitamin D levels in fighting COVID-19.

The study randomly assigned participants with asymptomatic or mild COVID-19 to two groups: one receiving vitamin D3 liquid and the other receiving a placebo (distilled water). The treatment group comprised 16 individuals, while the control group had 24. All participants were found to be vitamin D deficient (<20 ng/ml), were given standard care, and did not have significant comorbidities.

The trial utilized oral cholecalciferol, which is less potent than calcifediol used in the earlier study. This means that cholecalciferol takes longer to elevate vitamin D3 levels. Nonetheless, participants in the treatment group received high doses of cholecalciferol at 60,000 IU daily for a week. On day seven, those who remained deficient continued with the same dosage for another week, while those who had sufficient levels switched to 60,000 IU weekly.

Since over 80% of COVID-19 cases exhibit mild or no symptoms, achieving early viral clearance could significantly curb the spread of the virus.

On day 14, those in the treatment group had their vitamin D3 levels restored to approximately 50 ng/ml, while the placebo group remained deficient. Remarkably, 62.5% (10 out of 16) of those treated tested negative for SARS-CoV-2 within 21 days, compared to only 20.8% (five out of 24) in the placebo group, demonstrating a statistically significant improvement without any signs of vitamin D toxicity.

“In this pioneering intervention study using cholecalciferol for asymptomatic and mildly symptomatic SARS-CoV-2 positive individuals, we discovered that a larger percentage of patients achieved SARS-CoV-2 RNA negativity with high-dose vitamin D supplementation compared to those who remained vitamin D deficient,” the researchers concluded.

Vitamin D3 deficiency is alarmingly common, affecting around 35% of adults in the U.S. and up to 80% in countries like Pakistan, India, and Bangladesh. These figures are even more pronounced among older populations, with deficiencies reaching 61% in the U.S. and 96% in India. This widespread deficiency raises an intriguing question: could vitamin D3 deficiency, which prolongs the duration individuals carry the coronavirus, be contributing to the spread of COVID-19? While concrete data to support this claim is currently lacking, it remains an avenue worth exploring.

In summary, vitamin D3 plays a vital role in regulating the immune response and managing the renin-angiotensin system (RAS), both of which are crucial in preventing severe outcomes in COVID-19 patients. Several studies have linked low vitamin D3 levels to a higher risk of SARS-CoV-2 infection and unfavorable clinical outcomes.
